ABSTRACT

The article describes the features of the course of pregnancy and childbirth in women with a new coronavirus infection. Aim. To consider the features of the course of pregnancy and its outcome in women infected with a new coronavirus infection, reflected in current literary sources. Materials and methods. Scientific literature and reviews in the WoS and Scopus systems. A review of scientific papers was carried out. Different cases of pregnancy and childbirth under the condition of infection of a woman with the Covid-19 virus are considered in the scientific literature. Results. Physiological changes that occur in a woman's body during pregnancy make her body more vulnerable to various pathogens. The experience of managing pregnant women in the conditions of a pandemic of a new coronavirus infection allows us to form a the-oretical idea of the susceptibility of the body of a woman infected with Covid-19 and the features of possible complications caused by the course of the disease for the mother and fetus. Conclusion. The course of the disease in pregnant women with a confirmed new coronavirus infection is on average similar to the course of the disease in non-pregnant women with pneumonia caused by infection with the SARS-Cov-2 virus. However, pregnancy management of patients infected with the SARS-Cov-2 virus requires special attention and individualization based on obstetric indications and characteristics of the condition of the mother and fetus. © 2023, Media Sphera Publishing Group. ABSTRACT

The article conducts a study of the effect of new coronavirus infection (SARS-CoV-2) on the functional state of newborns born from women with a confirmed diagnosis of COVID-19 in the Stavropol region. Objective. To study the clinical and laboratory manifestations of SARS-CoV-2 in newborns born from women with a confirmed diagnosis of COVID-19 and women who suffered during pregnancy and recovered. Material and methods. A retrospective analysis of the diseases of 162 pregnant women hospitalized in the Stavropol region was carried out. The core of the study showed 81 women diagnosed with a new SARS-CoV-2 coronavirus infection. A retrospective analysis of cases of illness was carried out in 81 newborns born to women from the original group. Results. It was found that 52/81 (64.2%) women were delivered through the natural birth canal, 29/81 (35.8%) of the women in labor due to the increase in respiratory failure and, as a result, fetal hypoxia, were delivered by ACS. In 7/81 (8.64) children, according to pulse oximetry, the expected content of O2 in the blood was detected. In children, there is an increase in C-reactive protein and other inflammatory mediators with COVID-19, in contrast to large ones. They have extremely common leukopenia and lymphopenia. To a lesser extent, the concentration of the concentration of subpopulations of T-lymphocytes. Conclusion. In 2022, the existence of transmission of coronavirus from mother to fetus has not been proven. In this regard, the management of pregnant women should be individualized on the basis of taking into account obstetric indications, and the management of newborn children should be carried out, depending on the characteristics of the clinical case, together with pediatricians and, if necessary, with the staff of ICU. © 2022, Media Sphera Publishing Group. ABSTRACT

Currently, intensive study of the clinical and epidemiological features of COVID-19 is continuing. In this regard, the development of new means of its prevention and treatment is becoming increasingly relevant. The most vulnerable category of people, along with older people, is pregnant women. Due to their low immune reactivity, they are more susceptible to severe complications than others. In this article, we conducted a retrospective analysis of 280 case histories of pregnant women living in Republics of the North Caucasus Federal District of the Russian Federation (the Republic of Ingushetia, the Republic of Dagestan and the Republic of North Ossetia-Alania) who received medical care in 2021 with an officially confirmed diagnosis of mild and moderate COVID-19 coronavirus infection. Analyzing the pregnancy outcomes in patients with COVID-19, we found that 15.3% of women were delivered through the natural birth canal, 10.7% of women in labor due to an increase in respiratory failure and, as a consequence, fetal hypoxia, were delivered by caesarean section. 2.5% of cases ended in spontaneous abortion in the first trimester. 71.4% of pregnant women who underwent COVID-19 were hospital discharged with progressive pregnancy at different gestation periods. As a result of the work carried out, we analyzed a number of clinical and laboratory data in pregnant women with COVID-19 in dynamics assessed the course of the disease in different periods of pregnancy by region and assessed the effectiveness of pharmacotherapy.

ABSTRACT

By the beginning of 2022, the pandemic of the new coronavirus infection SARS-CoV-2 continues its escalation around the world, acquiring new strains and, accordingly, changes in the clinical picture of the disease. The most vulnerable category of people, along with the elderly, are pregnant women and women in labor due to low immune resistance. Currently, insufficient studies have been conducted on the effect of COVID-19 on the course of pregnancy and, especially, on the effect of coronavirus infection on the functional state of newborn children born to women with a confirmed diagnosis. We conducted a retrospective analysis of 391 case histories of pregnant women with a confirmed diagnosis of SARS-CoV-2 and 148 live infected newborns who received medical care from the 1st to the 4th quarter of 2021 in the Republics of the North Caucasus Federal District: the Republic of Ingushetia, the Republic of Dagestan and the Republic of Northern Ossetia-Alania. The results of the analysis revealed the features of the course, outcomes of pregnancy and assessment of the health of newborn children, as well as the effectiveness of the therapeutic strategy in COVID-19.

ABSTRACT

The Electronic Clinical Pharmacologist (ECP) is a Medical Decision Support System (MDSS). This system is based on the Unified Medical Knowledge Base (UMKB), which is updated and updated as new medicines are released and specialized publications are published in peer-reviewed biomedical scientific journals. ECP helps to reduce the risks of medical errors and complications in clinical practice. When using ECP, the number of side effects from the use of medicines decreases, the patient's admission time is reduced, the quality of medical care is improved, the costs of the medical organization for the purchase of medicines are reduced, all this is carried out due to more rational prescriptions of the doctor. The ECP takes into account the personalized approach of drug therapy. Based on Stavropol State Medical University and medical universities of the North Caucasus Federal District, a questionnaire of students was conducted among students of 3-6 courses, as well as testing of the ECP application to compare treatment standards (clinical recommendations) according to the clinical recommendations of the Ministry of Health (outpatient, inpatient treatment) and self-treatment of students and their relatives. ABSTRACT

This scientific article describes various neurological problems faced by patients with coronavirus infection. The article also contains information of a recommendatory nature for the treatment of cognitive disorders. In particular, the effective positive effect of the popular drug citicoline is described. According to various clinical studies, citicoline provides a statistically significant moderate but stable improvement in memory and helps to reduce the severity of behavioral disorders in patients with cognitive disorders of various etiologies. ABSTRACT

The developed HoloDoctor software package allows performing surgical operations in real time. It should be emphasized that the operation time on the HoloDoctor PC is reduced by 20-30% compared to traditional methods. The performance of HoloLens glasses with our program is high, while allowing the doctor to save time. The complex provides diagnostics, therapy planning, treatment adjustment as needed and minimizes the risk of medical error. HoloDoctor can be used in educational practice by integrating it into educational programs for medical specialties.

ABSTRACT

The aim of the study was to determine the effect of the inhibitor of janus kinase - baricitinib ("Olumiant") on the course of COVID-19. This drug baricitinib is able to suppress the systemic inflammatory response, which is one of the common causes of death in COVID-19, is an urgent problem of the study. The study was carried out in the Regional Specialized Budget Infectious Diseases Hospital of Stavropol (Russian Federation, Stavropol Region). In a multivariate analysis, it was shown that the use of baricitinib was associated with a decrease in the frequency of the primary endpoint of death/need for invasive lung ventilation. The use of baricitinib was quite safe, but in some patients there was an increase in the level of transaminases. No cases of hypercoagulation have been reported while taking baricitinib. It was found that patients with a normal BMI are more sensitive to therapy than those who are overweight.75% of patients in the first group had a temperature above 38.0 degrees C. However, against the background of taking the standard treatment regimen and baricitinib, it was noted that the temperature stabilized during the day and did not rise again. Against the background of taking baricitinib, the elimination of the phenomena of respiratory failure and the refusal of an oxygen mask occurred 3.1 times more often than without baricitinib. The authors concluded that the use of baricitinib can be considered as an additional therapy for moderate forms of pneumonia in patients over 65 years or in patients with concomitant chronic diseases in order to suppress the reactions of systemic inflammation and the development of serious respiratory and other system lesions caused by COVID-19.

ABSTRACT

The purpose of this scientific article is to show the possibility of using the HoloDoctor software and hardware complex, which includes the following modules: the anatomical atlas module, the data analysis module and the surgical intervention simulation module. HoloDoctor allows us to perform surgical operations in real time. The time of operations using the HoloDoctor is reduced by 20-30% compared to traditional methods. The developed complex provides diagnostics of treatment planning, correction of the treatment plan, minimizing the risk of medical error. The developed product can be used in educational practice by introducing it into educational programs of medical specialties.

ABSTRACT

This article presents an analysis of available scientific data on the morphology and nanostructure of the COVID-19 coronavirus. Possible mechanisms of influence of nanosilver particles on the coronavirus are considered. Models of nanosilver complexes with spike protein of coronavirus amino acids were constructed using computer quantum-chemical modeling. The values of electron density distribution, highest occupied molecular orbital, lowest unoccupied molecular orbital, and electron density distribution gradient for each constructed model are obtained. As a result of quantum chemical modeling, it was found that silver nanoparticles can interact with the following amino acids: Proline, glutamine, lysine, arginine, asparagine, histidine, glutamic and aspartic acids, tryptophan, and cysteine, which is due to the presence of additional -NH2, -NH, -SH and -COOH groups in these amino acids that are not involved in the formation of a peptide bond. The freedom of additional groups makes it possible to interact with nanosilver. Analysis of the obtained data showed that the most energy-efficient interaction is the formation of the "tryptophan-nanosilver" complex (E= - 5856.83 kcal/mol). Based on the findings of quantum chemical calculations, the most stable complex is the "cysteine- nanosilver" (Delta E = 0.16 a.u). Copyright (C) 2013 - All Rights Reserved - Pharmacophore

ABSTRACT

The study of the etiology of acute respiratory infections in 4022 hospitalized children aged 1 month to 14 years revealed the involvement of coronavirus in 12.4% of cases (500 children) more often in combination with other pathogens. In the case of coronavirus infection, moderate forms of the infectious process prevail mainly in the form of rhino-pharyngo-tracheitis against the background of subfebrile and febrile temperatures with moderate symptoms of intoxication.
